ALBANY FAMILY MEDICINE, COMMUNITY CARE PHYSICIANS

Albany Family Medicine welcomes patients of all ages, from infants through adulthood, offering family medicine and OB/GYN services, osteopathic manipulation, as well as on-site blood drawing services. They provide office-based and inpatient care at St. Peter’s Hospital in Albany and Albany Medical Center for our obstetrics patients. They offer extended evening hours during the week and both in-person and virtual appointments for your convenience. They are located directly across the street from Albany Med’s ER. Patients can park in the parking garage (405 Myrtle Avenue in Albany) and enter the 4th floor to get to their office.


CAPITAL REGION FAMILY HEALTH CARE

Providing routine care for illness, injury and chronic conditions for patients of all ages, Capital Region Family Health Care is conveniently located in East Greenbush, just minutes from Exit 9 on I-90. Their expert and caring providers offer pediatric exams, routine immunizations and preventive inoculations, preventive health and wellness care, and patient education. The practice also offers onsite X-ray and laboratory/phlebotomy services.


COHOES FAMILY CARE

Cohoes Family Care offers a full range of health care services for the entire family, including: routine pediatric exams (well-baby and well-child checkups, school, camp and sports physicals), adolescent medicine, preventive immunizations and precautionary inoculations. Cohoes Family Care also offers the MOMS Program to support pregnant women through enhanced casework and nursing services.

GENERAL PEDIATRICS AT ALBANY MED

The general pediatrics team at Albany Med offers a broad range of health care services. For expectant parents, we offer prenatal visits and bedside visits for new mothers. Newborns in our program are followed through infancy and their adolescent years with vaccinations and annual wellness visits, as well as referrals to specialty services. Their pediatric health care professionals offer treatment for developmental and behavioral health conditions and specialize in pediatric neurology, cardiology, gastroenterology, oncology, and hematology, among other specialties.

Koinonia is a NCQA certified Level III Patient Centered Medical Home (PCMH) that provides compassionate thorough care coordination and management for all in need of health care (with a focus on low-income residents and the underserved in our community). Koinonia provides comprehensive primary and preventive care services to children and adults such as annual wellness visits, work PEs, immunizations, chronic disease management for HTN, diabetes, obesity, pain management, asthma, etc. They also provide on-site testing for hemoglobin A1c, lipid panel, pregnancy tests, strep tests, flu tests, urine screening and EKGs. Finally, they partner with organizations such as Trinity Alliance to assist patients with non-medical needs such as housing, access to food, transportation issues, etc.

St. Peter’s Pediatrics provides pediatric medicine for newborns through age 18, including well-child visits (routine, school, sports and camp physicals), immunizations, adolescent health, and sick visits by appointment. The center also provides care for ADHD and hospital/ER follow-up. Clinical social workers are available for consultation, assessment and referrals. After hours, patients have access to a health care provider by calling our main number and leaving a message with the answering service.

ST. PETER’S RENSSELAER HEALTH CENTER

The practice also offers refugee health assessments in conjunction with the New York State Department of Health. Clinical social workers are available for consultation, assessment and referrals. TROY PEDIATRIC HEALTH CENTER

This multi-provider office is centrally located in downtown Troy and offers high-quality pediatric medicine for children ages newborn to 19 years. Services include: routine physicals, sports physicals, urgent sick visits, medication management, and social work intervention. After-hours clinical advice is available via our phone triage nurse. The office works with Burdett Care Center and is responsible for the admission, discharge and care of newborns in this maternity hospital, located at Samaritan Hospital.
